Midwest Institute for Minimally Invasive Therapies, PC (MIMIT) is a private practice physician group in
the Chicago land area.
MIMIT operates under the following set of principles in terms of
the way it collects and uses visitor data from its Web site,
mimit.org:
- The only information MIMIT obtains about individual visitors
is that which is supplied voluntarily by visitors.
- Personally identifiable information (name, e-mail or home address,
etc.) is not disclosed to anyone other than the physicians and
staff of MIMIT, unless visitors indicate
that the information may be shared with a designated group of
individuals. (For example, with other physicians involved in the
person's medical care.)
An IP address is a number automatically assigned to your computer
whenever you access the Internet. All computer identification on
the Internet is conducted with IP addresses, which allow computers
and servers to recognize and communicate with each other. MIMIT collects
IP addresses in log files in order to conduct system administration,
evaluate aggregate data such as total traffic to the site, and conduct
site analyses.
If a visitor requests (clicks on links to) pages within mimit.org,
the mimit.org servers enter the visitor's IP address into a log.
To maintain visitor anonymity, MIMIT does not associate IP addresses
with records containing personal information. However, we do use
IP addresses to identify any visitors who refuse to comply with
our in-house rules or terms of service, and to identify visitors
who threaten our service, site, customers or others.
Cookies are pieces of information that a Web site transfers to
an individual's hard disk for record keeping purposes. mimit.org
does not use cookies.
In order to provide visitors with greater value, MIMIT provides
links from mimit.org to other Web sites. Please be advised that
MIMIT
is not responsible for, nor does it maintain information about,
the privacy policies used by these other sites. When visitors leave
mimit.org to visit third party sites connected via external links,
they should carefully read the privacy policies of each site to
understand how their personally identifiable data will be collected
and used. Destination links are provided only for your convenience;
access them at your own risk.
At some point in time, MIMIT may provide visitors and patients with
live chat events, forums, and message boards. Any information disclosed
in areas such as these becomes public information, so use caution
when deciding to disclose your personal information. Do not share
any data about yourself that you don't want anyone else to read.
At some time in the future, MIMIT may provide visitors the opportunity
to opt-in and subscribe to email communications, mostly in the form
of an e-newsletter. All recipients of such a communication provide
their contact information at will; in other words, they give their
permission to receive such communications.
Further, subscribers have the freedom to opt-out from these communications
at any time, by following the instructions at the end of each communication.

MIMIT takes every precaution to protect visitor information. One
of these precautions is the use of encryption software for electronic
messages that are transmitted from visitors to MIMIT physicians and
staff.
Data and information is transmitted over a secure channel, then
"unencrypted" in a secure database. MIMIT physicians and
staff are then alerted that messages are waiting in this secure
database, then retrieve them using an encrypted channel as well.
This security measure greatly minimizes the chance that messages
will be intercepted by third parties not affiliated with MIMIT. 128 bit encryption is used; the highest
available, and the level recommended by Center for Medicare and
Medicaid Services (the federal department that administers Medicare
and Medicaid).
In addition, MIMIT maintains strict policies about which staff and
other parties may view patient identifiable information. If you
